Mental Health Nursing: Therapeutic Communication and Safety

Mental health nursing combines therapeutic relationships, structured assessment, risk recognition, medication monitoring, psychosocial support, and multidisciplinary care.

Therapeutic Relationship

Use respect, empathy, consistency, boundaries, active listening, and non-judgmental communication. The goal is to understand the patient’s experience while maintaining professional limits.

Communication Skills

Helpful techniques include open-ended questions, reflection, clarification, summarizing, silence, and acknowledging feelings. Avoid arguing, false reassurance, threats, and dismissive language.

Mental Status Assessment

Common domains include appearance, behavior, speech, mood, affect, thought process, thought content, perception, cognition, insight, and judgment.

Risk Assessment

Ask directly about self-harm, suicide, harm to others, severe agitation, command hallucinations, substance use, and access to means when clinically indicated. Escalate concerns according to local policy.

De-escalation

Maintain a calm approach, reduce stimulation, respect personal space, use clear choices, avoid confrontation, and ensure staff safety. Seek assistance early when risk is increasing.

Medication Monitoring

Observe therapeutic response, adherence barriers, sedation, movement-related effects, metabolic changes, cardiovascular effects, and other adverse reactions according to the medication involved.
